Overview

The Quality Review Specialist will be a key member of the Adult Health Home team. The purpose of this position is to ensure the quality of services provided and documentation

 

This is a full time, 35 hr/week Monday - Friday position.

Responsibilities

  1. Ensure the completion of annual comprehensive assessments inclusive of medical, behavioral, rehabilitation, social service and long term care needs.
  2. Review patient centered plans of care.
  3. Review notes to ensure the delivery of all aspects of care coordination including work with service providers and health plans to provide necessary care for clients.
  4. Review notes to ensure the provisions of comprehensive transitional care including follow up with hospitals, ER, residential rehabilitation, following admissions and discharges to outpatient providers.
  5. Review all notes prior to submission to the Health Home to confirm billable status and lack of errors.
  6. Fix minor errors found upon review and communicate more significant errors to the Care Manager supervisor.
  7. Complete all required documentation in timely and through manner.
  8. Maintain guidelines established by the funding sources and Catholic Charities policies and procedures.

Qualifications

Education and/or Experience Required at Entry:  Bachelor’s degree in Nursing or Human Services related field and/or CASAC.  A minimum of two (2) years experience providing direct services and/or care coordination to persons diagnosed with mental disabilities, developmental disabilities, alcoholism or substance abuse. A Master’s degree in a related field may substitute for one year’s experience. Demonstrated ability to work with people of diverse backgrounds. Thorough working knowledge of local and regional public and private resources, including Social Security, Department of Social Service Adult Protective Unit, Income Assistance Unit and various other agencies and offices as needed. Experience documenting in an EHR strongly preferred.

 

Licenses/Certificate/Clearances: Valid N.Y.S. Drivers License, N.Y.S. Child Abuse Registry clearance.

Skills/Abilities: Computer skills. Creative problem solving. Negotiation skills.

Working Conditions and Environment/Physical Demands: Mobility sufficient to drive to and participate in meetings at client’s homes and/or community facilities which may not meet Federal accessibility standards for disabled individuals.

 

Required Trainings: VIRTUS, Corporate Compliance, Defensive Driving
