The Care Manager will coordinate and implement all case management services to pregnant women residing in Onondaga County. Case Management services include: assessments, development of a service plan, service monitoring and follow-up. Conduct one-on one health education on topics including smoking cessation, nutrition, prenatal care, SIDS risk reduction and inter-conception care. Provide screening services for domestic violence and perinatal depression.
Bachelor's Degree preferred. Associates degree with two years experience in providing home based services with vulnerable populations required. Experience working with expectant mothers and parents of children under two years of age preferred. Home-visiting experience preferred. Valid NYS driver's license. Ability to utilize electronic database system. Proficiency in MS Outlook and MS Office. Customer service skills and oral and written communication skills.
