    Responsibilities

    The Care Manager will provide comprehensive care management, care coordination, comprehensive transitional care, youth & family support & health promotion. Referrals to community & social support services. Will provide comprehensive assessment and reassessment of youth as needed inclusive of medical, behavioral, rehabilitation, social service and long term care needs. Ensure the delivery of all aspects of care coordination including work with service providers and health plans to provide necessary care for youth. Refer youth and family members as needed to appropriate community supports and help connect to those services.

    Qualifications

    A Bachelor’s of Science or Arts degree or NYS licensure and current registration as a Registered Nurse with two years relevant experience; or a Master’s Degree with one year relevant experience required.  Demonstrated ability to work with people of diverse backgrounds required. Degrees in Health or Human Services field preferred; experience working with an electronic health record system preferred. Experience working with children and families preferred. Thorough working knowledge of local and regional public and private resources needed.
