    Responsibilities

    The Quality Review Specialist will ensure the completion of annual comprehensive assessments inclusive of medical, behavioral, rehabilitation, social service and long term care needs. Review patient centered plans of care. Review notes to ensure the delivery of all aspects of care coordination including work with service providers and health plans to provide necessary care for clients. Review notes to ensure the provisions of comprehensive transitional care including follow up with hospitals, ER, residential rehabilitation, following admissions and discharges to outpatient providers. Review all notes prior to submission to the Health Home to confirm billable status and lack of errors. Fix minor errors found upon review and communicate more significant errors to the Care Manager supervisor. Complete all required documentation in timely and through manner. Maintain guidelines established by the funding sources and Catholic Charities policies and procedures. Supervise care managers by providing weekly supervision and assistance as needed. Complete HR forms, annual employee evaluations and approve time in ADP.

     

    Qualifications

    Bachelor’s degree in Nursing or Human Services related field and/or CASAC.  A minimum of two (2) years experience providing direct services and/or care coordination to persons diagnosed with mental disabilities, developmental disabilities, alcoholism or substance abuse. A Master’s degree in a related field may substitute for one year’s experience. Demonstrated ability to work with people of diverse backgrounds. Thorough working knowledge of local and regional public and private resources, including Social Security, Department of Social Service Adult Protective Unit, Income Assistance Unit and various other agencies and offices as needed. Experience documenting in an EHR strongly preferred.Valid N.Y.S. Drivers License, N.Y.S. Child Abuse Registry clearance Ability to utilize electronic database system. Proficiency in MS Outlook, and MS Office. Customer service skills. Oral and written communication skills
