    Responsibilities

    The Care Manager will maintain contact with program participants as necessary. Maintain documentation of all services contemporaneously. Coordinate services and develop plans using a culturally competent, respectful, person-centered, family-driven model.  Connect the individual and family with appropriate services to meet needs and identified Valued Outcomes. Assist the individual in maintaining benefits and entitlements.  Maintain the MSC record in accordance with CCOC and licensing agency requirements. Report known or suspected incidents and allegations of abuse/neglect to the MSC Program Manager. Follow all appropriate agency policy and procedures. Share in “on call” rotation as needed. Adhere to the philosophy of Catholic Charities and practice behaviors consistent with the National Alliance for Direct support Professionals (NADSP) Core Competencies, which include: Putting Individuals First; Building and Maintaining Positive Relationships; Demonstrating Professionalism; Supporting Good Health; Safety; Supporting the individual to live in the home of their choice, and being active and productive in their community.

     

    Qualifications

    Associate’s Degree in Health or Human Services.  One year of experience working for individuals with developmental disabilities, or one year of providing case management to any population. Valid N.Y.S. Driver’s License, registered and insured vehicle, N.Y.S. Child Abuse Registry clearance, OPWDD fingerprinting and background clearance.
