The Registered Nurse is a member of the services team for clients affected by Chronic Homelessness and Co-Occurring Mental Health and Substance Use disorders residing in the Catholic Charities Men's Shelter. RN will provide medical case management services to include: assessing clients for a range of health care needs and facilitate linkage and on-going connection to relevant urgent, primary and specialty health services; advocate for clients' access to community and health care resources and services; consult and collaborate with community providers to ensure continuity of care; provide first aid and triage; and tuberculosis screenings.
Associate’s Degree with current and active NYS RN licensure with a minimum of 1 (one) year relevant experience in the field of nursing and experience working with similar population. Valid N.Y.S. Drivers License and registered vehicle and N.Y.S. Child Abuse Registry clearance required.
