    Responsibilities

    The Care Manager will identify eligible clients by working with case workers in the agency and community. Educate providers on eligibility criteria and work with lead health home to determine eligibility. Follow up with eligible clients to discuss enrollment. Complete a comprehensive health assessment of each client. Assess medical and social service needs. Work with the client to create treatment goals and an individualized, client- centered plan of care. Refer client to needed services to support care plan and treatment goals. Coordinate with primary care physicians, specialists, and other health care providers and assist with medication reconciliation. Monitor and support the client in attending medical appointments.  Assist with scheduling and transportation needs. Facilitate discharge planning from ER, hospital or rehabilitative setting to ensure care needs are met. Maintain guidelines established by the funding sources and Catholic Charities policies and procedures.

    Qualifications

    Bachelor’s Degree in Nursing or a Human Services related field required with three years’ experience working with vulnerable adults or an equivalent combination of experience and education. Demonstrated ability to work with people of diverse backgrounds. Valid N.Y.S. Driver’s License, N.Y.S. Child Abuse Registry clearance.
