    The Peer Navigator will maintain office hours at assigned non-profit partners as scheduled, and see refugees on a drop-in basis. Make home visits to referred refugees. During home visits or office hours, assess refugee’s wellbeing and successful community integration. Document wellbeing and/or concerns using a standardized assessment tool. Report concerns to the Program Manager, and to Case Managers as appropriate. As assigned, perform basic health care checks with refugees, including but not limited to blood pressure checks.  Record results and report concerns to the supervisor. Link refugees to appropriate health and mental health services utilizing the Refugee Health Coordinator at Northside CYO. Act as a support and peer advocate for refugees according to their individual needs. Accompany refugees to health and mental health appointments as necessary to assist with engagement to services. Attend monthly trainings with other Peer Navigators as scheduled. Interpret for health and mental health services as necessary. Maintain guidelines established by the funding source and Catholic Charities policies and procedures.


    This position does have the possibility of having Part-Time hours!


    Associates Degree in Human Services field preferred. HS Diploma with one (1) year relevant experience. Having the ability to engage with diverse families and respectful of cultural differences. Ability to write clear case notes in English. Fluency in English and at least one of the following lanuages: Farsi, Dari, Arabic, Karen, Burmese, Tigrinya, Swahili.
