The Program Manager will direct and supervise Care Managers and provide appropriate guidelines weekly. Review client charts monthly to ensure compliance with agency, Health Home, and DOH guidelines. Review Care Manager notes monthly to ensure delivery of high quality and billable services. Assist in recruiting, training and evaluating program staff. Maintain accountability for all assigned staff. Perform Outreach activities as necessary. Act as professional role model for staff and participants. Maintain guidelines established by the funding sources and Catholic Charities policies and procedures.
A Bachelor’s Degree and (2) two years relevant experience; or a Master’s Degree with (1) one year relevant experience required. Master's of Social Work and knowledge of Health homes preferred.
Demonstrated ability to work with people of diverse backgrounds required. Experience working with an electronic health record system preferred. Experience working with children and their families.
