The Caseworker will provide information and referrals to a caseload of 60-65 senior citizen clients referred by Onondaga County.
Scheduling in person home visits and completing in home assessment forms for new clients within 10 days of the screening.
Attending monthly EISEP Case Manager Meetings and in-services as scheduled by Department of Adult and Long Term Care Services.
Ensuring regular contact with county office including updates, observations and challenges.
Adhering to timely schedule of activities (assessments, re-assessments, home visits. phone calls, emails, and mailings etc.) as mandated by Onondaga County EISEP office.
Maintaining all client records in Peer Place (New York State Health Commerce Software System). Including entering all data. This includes contacts/units, visits, phone calls, emails, mailings and detailed client notes etc.
Provide monthly reports to the EISEP Project Director.
Bachelor’s Degree in a Human Services/ Social Work/Nursing required with a minimum of two (2) years experience working with senior citizens or an equivalent combination of experience and education. Knowledge of community resources. Experience working with the elderly and or senior citizens required.
Valid N.Y.S. Drivers License. Reliable registered and insured vehicle.
