Responsibilities

Essential Functions/Responsibilities:

The Program Director will provide directorial oversight to the Adult Health Home program and Elderly Services programs to ensure the provision of high-quality, person-centered services. Oversee the daily operations of the Adult Health Home and Elderly Services programs. Ensure that assigned programs are operating as designed and achieving stated outcomes across all sites. Ensure compliance with NYS DOH NYS OFA, and Onondaga County Office for the Aging regulations in service provision. Recruit, hire, train, supervise, and evaluate program staff. Provide weekly supervision to program managers. Ensure that program goals are met by frequently assessing compliance with funding source and CC policies and procedures including documentation, reporting, and auditing. Maintain positive working relationships with community partners, lead Health Homes, county, and state representatives. Act as liaison and and CCOC representative in the community. Work collaboratively with CCOC Finance Staff and Program Officer in managing budgets and grants.

Qualifications

Education and/or Experience Required at Entry: Bachelor's degree and 3-5 years related experience required. Master's degree in Social Work preferred. A minimum of 2 years supervisory experience. Thorough working knowledge of the Health Home program and services for the aging.

 

Licenses/Certificate/Clearances: Valid N.Y.S. Driver's License, NYS driver's license check, criminal background check, State Central Register Clearance.

 

Skills: Excellent computer skills, confidentiality, oral and written communication skills. Strong ability to work both independently and as part of a team.

 

Working Conditions and Environment/Physical Demands: Mobiligy sufficient to drive to and participate in meetings at clients' homes and/or community facilities which may not meet Federal accessibility standards for disabled individuals. Manual dexterity sufficient to work with children/adults in a challenging and active environment.

 

Required Training: VIRTUS, Corporate Compliance, Defensive Driving, Mandated Reporting, UAS Training for HCBS Services, MAPP Training, EPIC Training and Health connections (RHIO).
