Responsibilities

The Program Manager will direct and supervise Care Managers and provide appropriate guidelines weekly. Review client charts monthly to ensure compliance with agency, Health Home, and DOH guidelines. Review Care Manager notes monthly to ensure delivery of high quality and billable services. Assist in recruiting, training and evaluating program staff. Maintain accountability for all assigned staff. Perform Outreach activities as necessary. Act as professional role model for staff and participants. Maintain guidelines established by the funding sources and Catholic Charities policies and procedures.

Qualifications

Education and/or Experience Required at Entry: A Bachelor’s Degree and 2 years relevant experience; or a Master’s Degree with one year relevant experience required. Demonstrated ability to work with people of diverse backgrounds required. Experience working with an electronic health record system preferred. Experience working with children and their families. 

 

Licenses/Certificate/Clearances: Valid NYS Driver’s License, NYS driever's license check, criminal background check, NYS Central Registry clearance.

 

Skills/Abilities: Intermediate computer skills, confidentiality, oral and written communication skills.


Working Conditions and Environment/Physical Demands: Mobility sufficient to drive to and participate in meetings at client's homes and/or community facilities which may not meet Federal accessibility standards for disabled individuals. Manual dexterity sufficient to work with childre/adults in a challenging and active environment.

 

Required Trainings: VIRTUS, Corporate Compliance, Core Updates, CANS-NY, Mandated Reporter, UAS, RELIAS, Netsmart Care Manager, RHIO, PSYCKES.
