The Lullaby League Healthy Start Case Manager will provide Lullaby League educational services to pregnant women residing in Onondaga County. Coordinate and implement all case management services including: instruction of Lullaby League/Partners for Health Babies and Health Start Assessments, development of a service plan, service monitoring, and follow up.

Essential Functions/Responsibilities:

• Teach the four week Lullaby League/Partners for Healthy Babies infant care course at a variety of sites in the community.
• Maintain class attendance records and paperwork, and co-facilitate the parent support groups.
• Conduct follow up phone calls and home visits with clients as agreed upon by case manager and supervisor while maintaining client confidentiality at all times.
• Collaborate with Onondaga County Health Department to ensure immunications, WIC, lead screening, Early Intervention, and EI CARES are offered and refer to Health Start when additional case management is needed.
• Provide referrals, collaboration, and linkages to community resources and other organizations.
• Meet with supervisor weekly to discuss case progress, challenges, receive guidance and support, and attend monthly training meetings.
• Maintain guidelines established by the funding sources and Catholic Charities policies and procedures.

Education and/or Experience Required at Entry: Associates degree required in Human Services or related field with two years of case management experience, or combination of education and experience. Experience in facilitating educational groups with vulnerable populations preferred. Experience working with expectant mothers and parents of children under two years of age preferred.

Licenses/Certificate/Clearances: Valid N.Y.S. Drivers License, N.Y.S. Child Abuse Registry clearance.

Skills/Abilities: Ability to utilize electronic database system. Proficiency in Microsoft Office, including Outlook. Customer service skills. Oral and written communication skills.

Working Conditions and Environment/Physical Demands: Mobility sufficient to drive to and participate in meetings at client’s homes and/or community facilities which may not meet Federal accessibility standards for disabled individuals, Manual dexterity sufficient to work with children/adults in a challenging and active environment.

Required Trainings: VIRTUS, Corporate Compliance, Defensive Driving, Mandated Reporting.
