This position offers a challenging and exciting opportunity to be part of an innovative statewide program designed to integrate children’s physical, behavioral, and social health in creative and collaborative ways. It will afford you the opportunity to enhance your written communication and time management skills, connect you to a wide array of community providers in the medical, psychiatric, and educational fields, and allow you to make a tangible positive impact on one of our community’s most vulnerable populations.
The Care Manager will:
Education and/or Experience Required at Entry: A Bachelor’s degree or NYS licensure and current registration as a Registered Nurse with two years relevant experience; or a Master’s Degree with one year relevant experience required. Demonstrated ability to work with people of diverse backgrounds required. Degrees in Health or Human Services field preferred; experience working with an electronic health record system preferred. Experience working with children and families preferred. Thorough working knowledge of local and regional public and private resources needed.
Working Conditions and Environment/Physical Demands: Mobility sufficient to drive to and participate in meetings at client’s homes and/or community facilities which may not meet Federal accessibility standards for disabled individuals. Mobility sufficient to work in an office environment. Manual dexterity sufficient to operate office equipment including machines.
Licenses/Certificate/Clearances: Valid N.Y.S. Driver’s License, N.Y.S. Central Registry clearance.
Required Trainings: VIRTUS, Corporate Compliance, Core Updates.
