The Intensive Case Manager will provide intensive case management to refugees who are arriving in Syracuse, New York from various countries. Work as a team with the Resettlement Case Manager to meet the overall needs of refugees with significant mental health or medical vulnerabilities. Ensure linkage of vulnerable refugees to appropriate medical or mental health clinicians, following the guidelines and timelines of IOM/PRM when identified before arrival. Ensure that vulnerable refugees consistently attend medical and mental health appointments and arrange transportation as necessary. Provide supportive counseling and guidance as needed to help refugees acclimate and integrate into the community. Provide crisis intervention supports as needed.


Link refugees to additional support services as needs are identified. Assist with maintaining housing, public benefits, employment if appropriate, educational programs, and and other resources that will facilitate the refugees' full integration into the Syracuse community. Facilitate weekly support group for refugees, ensuring enrollment of a minimum of 15 participants. Maintain a caseload of at least 15 intensive case management clients. Maintain timely case notes, service plan reviews, assessments, and enrollments as outlined by USCCB/MRS. Maintain a positive working relationship with medical and mental health providers in the community. Develop new partnerships as needed.


Education and/or Experience Required at Entry: Associate degree in Human Services or related field with two years of experience working with individuals who speak English as a second language, refugees, and individuals of diverse backgrounds.


Licenses/Certificate/Clearances: Valid N.Y.S. Driver’s License, Child Abuse Registry clearance.


Skills/Abilities: Ability to utilize electronic database system. Proficiency in MS Outlook, and MS Office. Customer service skills. Oral and written communication skills.

Working Conditions and Environment/Physical Demands: Ability to lift and carry up to 20 pounds. Mobility sufficient to drive to and participate in meetings at client’s homes and/or community facilities which may not meet Federal accessibility standards for disabled individuals.


Required Training: VIRTUS, Corporate Compliance, Defensive Driving, Mandated Reporting.
