The Care Manager will identify eligible clients by working with case workers in the agency and community. Educate providers on eligibility criteria and work with lead health home to determine eligibility. Follow up with eligible clients to discuss enrollment. Complete a comprehensive health assessment of each client. Assess medical and social service needs. Work with the client to create treatment goals and an individualized, client- centered plan of care. Refer client to needed services to support care plan and treatment goals. Coordinate with primary care physicians, specialists, and other health care providers and assist with medication reconciliation. Monitor and support the client in attending medical appointments. Assist with scheduling and transportation needs. Facilitate discharge planning from ER, hospital or rehabilitative setting to ensure care needs are met. Maintain guidelines established by the funding sources and Catholic Charities policies and procedures.
Education and/or Experience Required at Entry: Bachelor’s Degree in Nursing or a Human Services related field required with three years of experience working in care coordination or an equivalent combination of experience and education. Demonstrated ability to work with people of diverse backgrounds. Thorough working knowledge of local and regional public and private resources, including Social Security, Department of Social Service Adult Protective Unit, Income Assistance Unit and various other agencies and offices as needed.
Working Conditions and Environment/Physical Demands: Mobility sufficient to drive to and participate in meetings at client’s homes and/or community facilities which may not meet Federal accessibility standards for disabled individuals. Mobility sufficient to work in an office environment. Manual dexterity sufficient to operate office equipment including machines.
Licenses/Certificate/Clearances: Valid N.Y.S. Driver’s License, N.Y.S. Child Abuse Registry clearance.
Required Trainings: VIRTUS, Corporate Compliance, Core Updates.
