The Care Manager will:

• In person contact with clients is a core component of Adult Health Home Case Management. Care managers are required to develop a relationship and actively engage individuals, evaluate their environment, and provide outreach efforts necessary to support the population.
• Provide comprehensive assessment and reassessment of clients as needed inclusive of medical, behavioral, rehabilitation, social service, long term care needs and home and community based services.
• Identify eligible clients by working with case workers in the agency and community. Educate providers on eligibility criteria and work with lead health home to determine eligibility.
• Complete and/or revise as needed patient centered plan of care.
• Prepare client crisis intervention plan and participate in its execution as needed.
• Ensure the delivery of all aspects of care coordination including work with service providers and health plans to provide necessary care for clients.
• Advocate for services and assist with scheduling of medical services, monitor and support attendance at these services.
• Ensure the provisions of comprehensive transitional care including follow up with hospitals, ER, residential rehabilitation, following admissions and discharges to outpatient providers.
• Provide patient and family support by including family members in client’s plan of care when appropriate and serving as a resource to client and family regarding patient and community programs.
• Refer clients and family members as needed to appropriate community supporters and help connect to those services.
• Complete all required documentation in timely and through manner as identified by the Department of Health.
• Maintain guidelines established by the funding sources and Catholic Charities policies and procedures.

Education and/or Experience Required at Entry: Bachelor’s Degree in Nursing or Human Services related field required with 3 years of experience working with vulnerable adults or an equivalent combination of experience and education. Demonstrated ability to work with people of diverse backgrounds.

Skills/Abilities: Basic computer skills, creative problem solving, negotiation skills, ability to navigate and utilize multiple Electronic Medical Records (EMRs).

Licenses/Certification/Clearances: Valid N.Y.S. Driver’s License, N.Y.S. Child Abuse Registry clearance.

Working Conditions and Environment/Physical Demands: Mobility sufficient to drive to and participate in meetings at client's homes and/or community facilities which may not meet Federal accessibility standards for disabled individuals.

Required Trainings: VIRTUS, Corporate Compliance, Defensive Driving, Mandated Reporting.
