Responsibilities

Essential Functions/Responsibilities:

 

Interview clients with personal and family adjustments, finances, employment, food, clothing, housing, and physical and mental impairments to determine nature and degree of problem
Provide case management services to a caseload of individuals to assist in maintaining housing and support participants as an active and productive member of their family and community.
Secure information such as medical, psychological, and social factors contributing to client’s situation, and evaluate these and client’s capacities.
Maintain a minimum of monthly communication with clients in order to assess housing needs. Maintain communication with landlords to ensure clients remain stably housed.  Caseworkers conduct annual inspections of participants’ homes in order to ensure the client is residing in a safe environment.
Transport clients, as needed, to housing related appointments.
Counsel clients individually, in family, or in other small groups regarding plans for meeting needs, and aid clients to mobilize inner capacities and environmental resources to improve social functioning.
Maintain guidelines established by the funding sources and agency policies and procedures.
Act as professional role model for staff and participants.

Qualifications

Education and/or Experience Required at Entry: Associate’s Degree in a Human Services related field required with three years of experience working with adults or an equivalent combination of experience and education. Demonstrated ability to work with people of diverse backgrounds.  Candidates with lived experience are preferred, and encouraged to apply. Must possess a valid NYS driver’s license and reliable vehicle.

 

Licenses/Certificate/Clearances: Valid N.Y.S. Driver’s License, registered and insured vehicle, State Central Register Clearance.

 


Working Conditions and Environment/Physical Demands:   Mobility sufficient to drive to and participate in meetings at client’s homes and/or community facilities which may not meet Federal accessibility standards for disabled individuals, Manual dexterity sufficient to work with children/adults in a challenging and active environment.
