Responsibilities

The Healthy Start Case Manager will primarily coordinate and implement all case management services to pregnant women residing in the city of Syracuse. Case Management services include: Assessments, development of a Service Plan, service monitoring, and follow-up.

 

Additional Responsibilities include;

  • Conducting weekly face-to-face contact with clients as agreed upon by case manager and supervisor while maintaining client confidentiality at all times.
  • Providing screening services for domestic violence, perinatal depression and complete the Client Assessment Form within the first forty-five days of contact with SHS’ recipient.
  • Conducting one on one health education on topics including smoking cessation, nutrition, prenatal care, SIDS risk reduction, and interconception care.
  • Facilitation of prenatal care appointments for SHS’ recipients and provide patient navigation services. Also, assisting in making appointments for postpartum and pediatric visits. Follow up will be routinely provided to ensure compliance to these appointments.
  • Collaboration with Onondaga County Health Department to ensure immunizations, WIC, lead screening, Early Intervention, and EI CARES are offered to each SHS’ recipient.
  • Providing referrals, collaboration, and linkages to community resources and other organizations.
  • Completion of required documentation and maintaining up to date case files.
  • Meeting with supervisor weekly to discuss case progress, challenges, receive guidance and support, and attend monthly training meetings.
  • Maintainence of guidelines established by the funding sources and Catholic Charities policies and procedures

 

Qualifications

Bachelor’s degree preferred. Associates degree with two (2) years experience in providing home based services with vulnerable populations required. Experience working with expectant mothers and parents of children under two years of age preferred. Home-visiting experience preferred.

 

Valid N.Y.S. Drivers License, N.Y.S. Child Abuse Registry clearance.

 

Ability to utilize electronic database system. Proficiency in MS Outlook, and MS Office. Customer service skills. Oral and written communication skills.
