Responsibilities

Essential Functions/Responsibilities:

 

  1. Maintain guidelines established by the funding sources and Catholic Charities policies and procedures.
  2. Responsible for scheduling in person home visits and completing in home assessment forms for new clients within 10 days of the screening.
  1. Maintain caseload of 60-65 individuals referred by Onondaga County EISEP staff.
  1. Attend monthly EISEP Case Manager Meetings and in-services as scheduled by Department of Adult and Long Term Care Services.
  2. Ensure regular contact with county office including updates, observations and challenges. 
  3. Adhere to timely schedule of activities (assessments, re-assessments, home visits. phone calls, emails, and mailings etc.) as mandated by Onondaga County EISEP office. 
  4. Maintain all client records in Peer Place (New York State Health Commerce Software System).  Including entering all data.  This includes contacts/units, visits, phone calls, emails, mailings and detailed client notes etc. 
  5. Securely maintain all client files/paper copies for mandated audits and walk-ins. 
  6. Make appropriate referrals to local agencies to ensure comprehensive care for all clients.  
  7. Provide monthly reports to the EISEP Project Director.

Qualifications

Education and/or Experience Required at Entry:  Associate’s Degree in a Human Services/ Social Work/Nursing required with a minimum of 2 years of experience working with senior citizens or an equivalent combination of education and experience required. Knowledge of community resources. Experience working with the elderly and or senior citizens required.

 

Licenses/Certificate/Clearances: Valid N.Y.S. Drivers License. Reliable registered and insured vehicle.

   

Skills/Abilities:  Customer service, oral and written skills.

 

 

 

Working Conditions and Environment/Physical Demands:   Mobility sufficient to drive to and participate in meetings at client’s homes and/or community facilities which may not meet Federal accessibility standards for disabled individuals.

 

Required Trainings: VIRTUS, Corporate Compliance, Confidentiality/HIPAA, Defensive Driving
