Responsibilities

Essential Functions/Responsibilities:
1. Provide comprehensive assessment and reassessment of youth as needed inclusive of medical, behavioral,
rehabilitation, social service and long term care needs.
2. Complete and/or revise as needed a CANS NY assessment and person centered Plan Of Care.
3. Prepare crisis intervention plan and participate in its execution as needed.
4. Ensure the delivery of all aspects of care coordination including work with service providers and health plans
to provide necessary care for youth.
5. Advocate for services and assist with scheduling of medical services, monitor and support attendance at these
services.
6. Ensure the provision of comprehensive transitional care including follow up with hospitals, ER and following
admissions and discharges to outpatient providers.
7. Ensure that all care provided is family and youth guided. Provide youth and family support by including family
members in the Plan Of Care when appropriate and serving as a resource to client and family regarding
available community programs.
8. Refer youth and family members as needed to appropriate community supports and help connect to those
services.
9. Complete all required documentation in timely and thorough manner.
10. Interface and collaborate with Department of Social Service Caseworkers, Catholic Charities program staff,
both medical and nonmedical, as well as other community providers.
11. Maintain guidelines established by the funding sources and Catholic Charities policies and procedures.

Qualifications

Education and/or Experience Required at Entry: A Bachelor’s of Science or Arts degree or NYS licensure and
current registration as a Registered Nurse with two years relevant experience; or a Master’s Degree with one year
relevant experience required. Demonstrated ability to work with people of diverse backgrounds required. Degrees
in Health or Human Services field preferred; experience working with an electronic health record system preferred.

Experience working with children and families preferred. Thorough working knowledge of local and regional
public and private resources needed.


Licenses/Certificate/Clearances: Valid N.Y.S. Drivers License, N.Y.S. Child Abuse Registry clearance.
Skills/Abilities: Strong computer skills. Creative problem solving. Negotiation skills, ability to communicate and
interact with people from diverse backgrounds.


Working Conditions and Environment/Physical Demands: Must possess mobility sufficient to drive to, and
participate in, meetings at client’s homes and/or community facilities which may not meet Federal accessibility
standards for disabled individuals; Manual dexterity sufficient to work with children/adults in a challenging and
active environment. Must have the ability to lift up to 10lbs

 

Required Trainings: VIRTUS, Corporate Compliance, Defensive Driving, CANS NY certification, Children’s Health
Home and Netsmart curriculum.
