Responsibilities

The Refugee Services Intensive Case Manager plays a vital role in meeting the needs of refugees with significant mental health or medical vulnerabilities, working in collaboration with the Resettlement Case Manager. They ensure the timely linkage of vulnerable refugees to appropriate medical or mental health clinicians, facilitate their attendance at crucial appointments, and provide supportive counseling and guidance to aid in their acclimation and integration into the community. Crisis intervention support is offered when necessary, and the case manager identifies and connects refugees to additional support services, assisting with housing, public benefits, employment, education, and other resources. Maintaining a caseload of intensive case management clients, the case manager upholds diligent record-keeping practices and fosters positive relationships with medical and mental health providers while actively seeking new partnerships to enhance services.

 

Full-time, $23 per hour, 35 hr workweek

Qualifications

Education and/or Experience Required at Entry:

  • Associate degree in Human Services or a related field.
  • Two years of experience working with individuals who speak English as a second language, refugees, and individuals from diverse backgrounds.
  • An equivalent combinationof education and experience will be considered.

Licenses/Certificates/Clearances:

  • Valid N.Y.S. Driver's License.
  • Child Abuse Registry clearance.

Skills/Abilities:

  • Ability to utilize electronic database systems.
  • Proficiency in MS Outlook and MS Office.
  • Strong customer service skills.
  • Excellent oral and written communication skills.

Working Conditions and Environment/Physical Demands:

  • Ability to lift and carry up to 20 pounds.
  • Mobility sufficient to drive to and participate in meetings at clients' homes and/or community facilities that may not meet Federal accessibility standards for disabled individuals.
