Responsibilities

The Program Manager will oversee, direct and supervise staff to ensure delivery of health home care management services. Supervise care managers by providing weekly supervision and assistance as needed. They will ensure the completion of annual comprehensive assessments inclusive of medical, behavioral, rehabilitation, social service and long term care needs and review notes to ensure the delivery of all aspects of care coordination including work with service providers and health plans to provide necessary care for clients. They will confirm billable status prior to submission to the Health Home. The Program manager will review notes to ensure the provisions of comprehensive transitional care including follow up with hospitals, ER, residential rehabilitation, following admissions and discharges to outpatient providers. Complete all required documentation in timely and through manner. Participate in team meetings at the program and agency level. They  will assist in recruiting, training and evaluating program staff. They will attend required webinars and trainings. Maintain guidelines established by the funding sources and Catholic Charities policies and procedures.

This Position if Full Time at 35 Hours a Week and Pay is $55,000.00 Per Year

Qualifications

Education and/or Experience Required at Entry:

  • Bachelor’s degree in Nursing or Human Services related field and/or CASAC.
  • A minimum of two (2) years’ experience providing direct services and/or care coordination to persons diagnosed with mental disabilities, developmental disabilities, alcoholism or substance abuse.
  • A Master’s degree in a related field may substitute for one year’s experience.
  • Demonstrated ability to work with people of diverse backgrounds.
  • Thorough working knowledge of local and regional public and private resources, including Social Security, Department of Social Service Adult Protective Unit, Income Assistance Unit and various other agencies and offices as needed.

Licenses/Certificate/Clearances:

  • Valid N.Y.S. Driver’s License,
  • NYS driver’s license check,
  • State Central Register Clearance.

Skills:

  • Intermediate computer skills,
  • confidentiality,
  • oral and written communication skills

Working Conditions and Environment/Physical Demands:

  • Mobility sufficient to drive to and participate in meetings at client’s homes and/or community facilities which may not meet Federal accessibility standards for disabled individuals,
  • Manual dexterity sufficient to work with children/adults in a challenging and active environment.
