The Family Support Program Associate will help facilitate the transport of homeless individuals and families residig in hotels. Conduct initial diversion/intake screening prior to contacting 211 to verify homelessness eligibility of individuals and families. Support the quarantine placement of COVID individuals and families in DSS-contracted hotel. They will respond to all incoming calls from clients and/or providers, complete a diversion screening with walk-in clients prior to calling 211 to confirm homelessness eligibility. Pick up clients and transport them to the appropriate shelter as determined by 211/DSS. Support quarantine of COVID clients through proper documentation prior to their hotel. Help with placement, transportation to quarantine, and providing food, hygiene items, and ensure that clients know the hotel rules. The Homless Program Associate will transport meals/food packages to families staying in the hotels, as needed. They will work with designated hotel agencies/shelters to provide a warm client hand off. Maintain and update transportation logs and documentation via shift notes Communicate with other shift Program Associates/Caseworkers/admin as needed while ensuring there is adequate staffing prior to leaving a shift. Observes all safety rules and regulations including use of personal protective equipment.
This position pays $18.00/ Hour. This is a PER DIEM ON CALL position.
