The Lullaby League Case Manager will provide Healthy Start Lullaby League educational services to pregnant women residing in Onondaga County. Coordinate and implement all case management services include: Instruction of Lullaby League/Partners for Healthy Babies and Healthy Start Assessments, development of a service plan, service monitoring, and follow up.
They will teach the four week Lullaby League/Partners for Healthy Babies infant care course virtually and at a variety of sites in the community. Maintain class attendance records and paperwork, and co-facilitate the parent support groups, and conduct follow up phone calls, home visits, and case management with clients while maintaining client confidentiality at all times. They will facilitate prenatal care appointments for clients and provide patient navigation services. Also, assist in making appointments for postpartum and pediatric visits. Follow up with be routinely provided to ensure compliance to these appointments. Conduct one on one health education on topics including smoking cessation, nutrition, prenatal care, SIDS risk reduction, and interconception care. The Case Manager will providing screening services for domestic violence and perinatal depression and complete all required assessments and data collection forms with Syracuse Healthy Start’s recipients. While collaborating with Onondaga County Health Department to ensure immunizations, WIC, lead screening, and Early Intervention Services are offered, they will provide referrals, collaboration, and linkages to community resources and other organizations. Case Managers will meet with supervisor weekly to discuss case progress, challenges, receive guidance and support, and attend m onthly team meetings. Maintain guidelines established by the funding sources and Catholic Charities policies and procedures. Perform all other tasks and duties as assigned.
This Position is Full Time at 35 hours a week and Pay is $23.00 Per Hour.
Education and/or Experience Required at Entry:
*As a subcontracting agency of the Onondaga County Health Department (OCHD), must meet all health standards for health department personnel.
