The Case Worker will conduct apartment inspections, assisting with clean-outs and cleanliness, and helping clients maintain safe, stable housing. The Case Worker will provide weekly case management, create client-centered goal plans, and connect clients to appropriate services, including mental health care, substance use treatment, medical care, education, and employment resources. The Case Worker will utilize harm reduction and motivational interviewing techniques to effectively support clients. The position also involves transporting clients to essential appointments such as grocery shopping, medical visits, and laundry.
Administrative duties include maintaining accurate client files and documentation, entering case notes into a data system, compiling monthly statistics, and ensuring compliance with agency policies and funding requirements. Attendance at staff meetings and trainings is mandatory.
This Position is Full Time at 40 Hours a Week. Pay For This Position is $23.50 Per Hour.
