The Program Manager will conduct intake/screenings on referred clients to determine eligibility and needs to be met by case management. Make decisions and recommendations regarding referred clients entrance into ESSHI/OMH project. They will provide support level supervision and oversee daily operations of a caseworker and after hour specialist. Conduct bio-psychosocial assessments and social histories. This includes securing information such as medical, psychological, and social factors contributing to client’s situation, and evaluating these and client’s capacities. Make decisions and recommendations regarding referred clients entrance into ESSHI/OMH Project. Refer clients to community resources and other organizations. Network with community service providers. The Program Manager will complete all necessary reporting forms for government agencies and funding sources (UCR’s, progress notes, referrals, and statistics). Maintain client referral logs and files. Provide support level supervision and oversee daily operations of a caseworker and after hour specialist, including weekly supervision meetings. Maintain appropriate communication with supervisors and other funding sources. Notify of any safety issues in a timely manner. All other duties related to housing individuals and helping them remain housed, including but not limited to furnishing and supplying apartments. Maintain guidelines established by the funding sources and Catholic Charities policies and procedures.
This is a Full Time Position at 35 Hours a Week. Pay For This Position is $58,500.00Per Year
