Responsibilities

The Program Manager assumes a leadership role by directing and supervising staff, providing them with clear guidelines and support. They diligently review client charts on a monthly basis to ensure compliance with agency, Health Home, and Department of Health (DOH) guidelines. Additionally, they carefully review care manager notes to guarantee the delivery of high-quality and billable services. The Program Manager actively participates in the recruitment, training, and evaluation of program staff, fostering a capable and motivated team. They maintain accountability for all assigned staff members and act as a professional role model for both staff and participants, setting high standards of professionalism and ethical conduct. In order to stay current with industry knowledge and best practices, the Program Manager attends required webinars and trainings.

Qualifications

A Bachelor degree in related field and a minimum of five (5) years experience working in care management/coordination or an equivalent combination of education and experience.

Demonstrated ability to work with people of diverse backgrounds. Thorough working knowledge of local and regional public and private resources, including Social Security, Department of Social Service Adult Protective Unit, Income Assistance Unit and various other agencies and offices as needed.

 

  • Licenses/Certificate/Clearances: Valid N.Y.S. Drivers License, N.Y.S. Child Abuse Registry clearance.
  • Skills/Abilities: Basic computer skills. Creative problem solving. Negotiation skills.
  • Working Conditions and Environment/Physical Demands: Mobility sufficient to drive to and participate in meetings at client's homes and/or community facilities which may not meet Federal accessibility standards for disabled individuals.
