Responsibilities

The Refugee Services Intensive Case Manager plays a vital role in meeting the needs of refugees with significant mental health or medical vulnerabilities. Ensure timely linkage of vulnerable refugees to appropriate medical and support services; vulnerable refugees include those with disabilities, single-headed households, elderly, LGBTQ, attached minors, and those with mental health issues or medical needs. Facilitate their attendance at crucial appointments and ensure that the clients consistently attend medical appointments and follow-up appointments, including following up on referrals to specialists, understanding and following doctor’s directions, including assisting with prescriptions and understanding medication, securing medical equipment and assisting with health insurance issues. Arrange and assist with transportation to medical appointments, as necessary. Assisting clients and showing them the ways to become self-sufficient while also being a key support and connector to resources. Provide supportive counseling and guidance to aid in their acclimation and integration into the community. Provide crisis intervention support when necessary. Assist families submit guardianship petitions on behalf of attached minors, as needed. Identify and connect refugees to additional support services, assisting with housing, public benefits, employment, education, and other resources. Work as a team with the Refugee Services staff to meet the overall needs of vulnerable refugees. Maintain a caseload of intensive case management clients, the case manager upholds diligent record-keeping practices, conducts online assessments and tracks progress with regular case noting. Fosters positive relationships with medical and mental health providers while actively seeking new partnerships to enhance services. Maintain timely case notes, service plan reviews, assessments, and enrollments as outlined by Catholic Charities USA. Other duties as needed.

 

This is a  FULL-TIME, 35-hour per week position. Pay is $23.00/hour.

 

Qualifications

Education and/or Experience Required at Entry:

  • Associate degree in Human Services or a related field with two years of experience working with individuals who speak English as a second language, refugees, and individuals from diverse backgrounds.
  • An equivalent combination of education and experience will be considered.

Licenses/Certificates/Clearances:

  • Valid N.Y.S. Driver's License.
  • Child Abuse Registry clearance.

Skills/Abilities:

  • Ability to utilize electronic database systems.
  • Proficiency in MS Outlook and MS Office.
  • Strong customer service skills.
  • Excellent oral and written communication skills.

Working Conditions and Environment/Physical Demands:

  • Ability to lift and carry up to 20 pounds.
  • Mobility sufficient to drive to and participate in meetings at clients' homes and/or community facilities that may not meet Federal accessibility standards for disabled individuals.
  • Manual dexterity sufficient to work with children/adults in a challenging and active environment.
