Responsibilities

The Intake Center Case Worker will Monitor and assist individuals and families who have been referred to or are requiring assistance from Catholic Charities to insure their safety and basic needs are being met. They will help homeless and/or housing vulnerable individuals find and maintain housing and access benefits/income. Through interviewing clients, the Case Worker will gather information regarding problems the client face such as: personal and family adjustments, individuals whose children may be placed in foster care, finances, employment, food, clothing, housing, and physical and mental impairments to determine nature and degree of problem. They will provide homeless/crisis casework to all COVID quarantine residents in hotels, secure information such as medical, psychological, and social factors contributing to client’s situation, and evaluate these and client’s capacities. The Case Worker will counsel clients individually, in family, or in other small groups regarding plans for meeting needs, and aid clients to mobilize inner capacities and environmental resources to improve social functioning.  They will meet with clients as required by funding sources as well as refer clients to community resources and other organizations. The Case Worker will complete all necessary reporting forms for government agencies and funding sources (UCR’s, progress notes, referrals, and statistics). Maintain client case files and Network with community service providers. Case Workers will provide base level crisis response to all COVID quarantine residents in hotels, Secure meal delivery/ensure daily meal provision and Monitor clients’ progress, or lack thereof and complete daily notes. They will maintain guidelines established by the funding sources and Catholic Charities.

 

This is a Full Time Position at 35 Hours a Week. Pay For This Position is 21.00 Per Hour

Qualifications

Education and/or Experience Required at Entry:

  • Associate Degree in a Human Services related field with three years of experience working with vulnerable individuals
  • A combination of education and relevant experience


Skills and Abilities:

  • Excellent oral and written communication skills.
  • Proficiency with Microsoft Word.


Licenses/Certifications/Clearances:

  • Valid NYS Driver’s License,
  • registered and insured vehicle,
  • NYS Child Abuse Registry Clearance
